Community Health Assessments

During the years 2017 and  2018 Two Rivers Public Health Department underwent a community health assessment process. The results of this process are presented in three different reports: a strategic plan, a community health assessment, and a community health improvement plan.  

Our Community Health Assessment is a report only filled with the information that we gathered.

County Health Rankings Data and State Community Health Assessment

The County Health Rankings & Roadmaps program is a collaboration between the Robert Wood Johnson Foundation (RWJF) and the University of Wisconsin Population Health Institute. The RWJF believes America can become a nation where getting healthy, staying healthy, and making sure our children grow up healthy are top priorities. 

Annual County Health Rankings measure vital health factors to rank each county within each state.  This ranking is done nationwide in order to compare counties in each state.  The rankings listed below show how the counties in the Two Rivers Public Health District relate to other counties in Nebraska.

Local Reports

  • In 2012 ,Two Rivers created a Community Health Improvement Plan based off of a collection of data from interviews of community members, focus groups, national databases, and internal meetings. This plan has been updated periodically to inform others of the progress toward the main goals of the plan.

  • This is a strategic plan created in 2016 by staff and Board of Health members in 2016.
